 in the ass it's called an enema... much like a douche but takes a whole lot more liquid... very messy but cleans you out real nice....
you can Google now to do that on medical / home remedy websites

but it you can't do that then the insert out to wear a condom for STD safety and for hygiene reasons...
at least that is my take on it.
